BCM20702A0 (BCM4352) bluetooth only working intermittently

I've successfully installed macOS 10.12.6 on my Lenovo Yoga 720 and everything mostly works (after swapping in a Lenovo-branded BCM4352 Wifi Card). However, I noticed bluetooth only works intermittently in macOS after power cycles/sleep-wake cycles (BrcmPatchRAM2 is installed).

I've attached DSDT and IOReg's. (beforesleep = working, cold boot; aftersleep = not working, after 1 sleep/wake; aftersleep-twice = working, after a 2nd sleep/wake)

WiFi on the BCM4352 works fine with no issues...
